Job Description

• Assist with reviewing and updating individual tailor re-enablement plans and risk assessments on a regular basis.
• To support clients with daily living tasks such as domestic cleaning, shopping, and assisting with medication.
• Assist clients with meal planning, preparation and cooking
• To escort clients to appointment’s, GP's, hospital and other required journeys
• Promote service user's privacy, dignity, choice, safety, confidentiality and independence.
